Understanding PCL Reconstruction (Excluding Implants)

Posterior Cruciate Ligament (PCL) reconstruction is a surgical procedure aimed at restoring stability and function to the knee joint after PCL injury. Unlike ACL (anterior cruciate ligament) injuries, which are more common, PCL injuries are less frequent but can still cause significant knee instability and discomfort. PCL reconstruction involves the repair or replacement of the damaged ligament using graft tissue to restore proper knee function.

Preparation and Procedure

  1. Preoperative Evaluation: Prior to surgery, the patient will undergo a comprehensive evaluation, including a physical examination, imaging studies (such as MRI), and assessment of knee function to determine the extent of PCL injury and plan the surgical approach.

  2. Anesthesia: PCL reconstruction is performed under general anesthesia to ensure the patient’s comfort and pain control during the procedure.

  3. Arthroscopic Examination: Small incisions are made around the knee joint, through which an arthroscope (a thin, flexible tube with a camera) and surgical instruments are inserted. The surgeon uses the arthroscope to visualize the inside of the knee joint and assess the extent of PCL injury.

  4. Graft Harvesting: A graft tissue is harvested from either the patient’s own body (autograft) or a donor (allograft) to serve as a replacement for the damaged PCL. Common graft sources include the hamstring tendon, quadriceps tendon, or patellar tendon.

  5. Graft Placement: The harvested graft tissue is then secured in place using screws or other fixation devices to reconstruct the PCL. The graft is positioned to mimic the natural anatomy and function of the original ligament, restoring stability to the knee joint.

  6. Closure: After the reconstruction is completed, the incisions are closed with sutures or surgical staples, and the knee is bandaged to promote healing.
