Understanding Mastectomy
A mastectomy is a surgical procedure to remove one or both breasts, partially or completely. It is most commonly performed to treat breast cancer but can also be done as a preventive measure for those at high risk of developing the disease. There are several types of mastectomy, each tailored to the patient’s specific condition and treatment goals.
Types of Mastectomy
Different types of mastectomy include:
- Total (Simple) Mastectomy: Removal of the entire breast, including the nipple, areola, and most of the overlying skin.
- Partial (Segmental) Mastectomy: Removal of the cancerous part of the breast tissue and a margin of healthy tissue around it.
- Modified Radical Mastectomy: Removal of the entire breast along with some of the lymph nodes under the arm.
- Radical Mastectomy: Removal of the entire breast, lymph nodes under the arm, and chest wall muscles under the breast. This procedure is rare and usually reserved for advanced cancer cases.
- Skin-Sparing Mastectomy: Removal of the breast tissue, nipple, and areola, but most of the skin over the breast is preserved, often used when immediate reconstruction is planned.
- Nipple-Sparing Mastectomy: Removal of the breast tissue but preservation of the nipple and areola, also used when immediate reconstruction is planned.
The Mastectomy Procedure
Mastectomy procedures are typically performed under general anesthesia. The steps involved include:
- Preparation: Patients receive pre-operative instructions, including fasting guidelines and medication adjustments.
- Surgical Technique: The surgeon makes incisions based on the type of mastectomy being performed and removes the necessary breast tissue and possibly lymph nodes.
- Duration: The procedure usually takes between 1 to 3 hours, depending on the complexity.
- Completion: Once the mastectomy is complete, the incisions are closed with sutures, and dressings are applied. In cases of immediate reconstruction, a plastic surgeon will begin the reconstruction process during the same operation.
Recovery and Post-Operative Care
Recovery following a mastectomy varies depending on the type of surgery and whether reconstruction was performed. Our team at Neo TrueNorth provides detailed post-operative care instructions, including:
- Pain Management: Recommendations for managing any discomfort with over-the-counter or prescribed pain medications.
- Wound Care: Instructions on keeping the incision clean and dry, changing dressings, and monitoring for signs of infection.
- Drain Care: Guidance on caring for any surgical drains that may have been placed to remove excess fluid.
- Activity Restrictions: Advice on avoiding strenuous activities and heavy lifting for a specified period.
- Physical Therapy: Referral to physical therapy to aid in recovery and improve range of motion.
- Follow-Up Care: Scheduling follow-up appointments to monitor healing, discuss pathology results, and plan any additional treatments if necessary.
Potential Risks and Complications
As with any surgical procedure, mastectomy carries certain risks and potential complications, including:
- Infection: At the incision site, which may require antibiotics.
- Bleeding or Hematoma: Collection of blood at the surgical site.
- Seroma: Collection of fluid at the surgical site.
- Pain or Numbness: In the chest or arm area.
- Lymphedema: Swelling due to lymph node removal.
